Memorial Hermann HPB Oncology Pathways

Twelve hepato-pancreato-biliary system roadmaps as working consoles. Each one answers a stage or gate with the standard, the reasonable alternative and what to avoid, with the grade and the paper behind every claim — and says plainly where the evidence does not reach.

Liver

Five hepatocellular and metastatic pathways, from the incidental lesion through the named tumours.

HCC Hepatocellular carcinoma Roadmap v0.9 · 5 stages · 79 sources · liver-directed matrix CRLM Colorectal liver metastases Roadmap v0.8 · 5 gates · 68 sources · liver-directed matrix NELM Neuroendocrine liver metastases Roadmap v0.7 · 9 gates · 40 sources · liver-directed matrix Adenoma Hepatocellular adenoma Roadmap v0.2 · 10 gates · 55 sources Incidental mass Incidental liver mass Roadmap v0.2 · 9 gates · 42 sources

Bile duct

Three biliary pathways. The mixed tumour sits here because, once the diagnosis is known, it is staged, resected and treated along biliary lines rather than hepatocellular ones — and because the error this group punishes is calling it HCC.

iCCA Intrahepatic cholangiocarcinoma Roadmap v0.8 · 10 gates · 53 sources · liver-directed matrix Perihilar CCA Perihilar cholangiocarcinoma Roadmap v0.7 · 7 gates · 47 sources cHCC-CCA Combined hepatocellular-cholangiocarcinoma Roadmap v0.2 · 10 gates · 69 sources

Pancreas

Four pancreatic and periampullary pathways.

PDAC Pancreatic ductal adenocarcinoma Roadmap v0.3 · 9 gates · 43 sources PNET Pancreatic neuroendocrine tumour Roadmap v0.2 · 9 gates · 39 sources Pancreatic cyst Pancreatic cystic neoplasm Roadmap v0.2 · 10 gates · 31 sources Ampullary Ampullary tumours Roadmap v0.2 · 10 gates · 41 sources

What this is, and what it is not

These pages are drafts under active development, prepared for multidisciplinary and institutional governance review. They are not ratified clinical policy, they have not been approved by the specialties named in them, and they must not be used for patient care decisions.

Content is a decision-focused evidence synthesis, not a systematic review. Every criterion drawn from OPTN policy, drug labelling, NCCN or device authorisation must be re-verified against the current source before any use. Clinical transplant candidacy and allocation eligibility are separate judgements.

Updated 15 October 2026: CRLM and iCCA HAI evidence revised; PDAC experimental liver-metastasis branch removed.
